Kutty Philips Football Academy (KPFA) Registration Terms and Conditions
By completing and submitting this registration form to Kutty Philips Football Academy (KPFA), the player and their parent/guardian (if under 18 years of age) hereby confirm and agree to the following terms and conditions:
1. Accuracy of Information
All information provided in this registration form is true, accurate, and complete to the best of our knowledge. We confirm that no part of the information was falsified or misrepresented.
2. Voluntary Participation
The registration with KPFA is done voluntarily. The player and/or their parent/guardian confirm they were not coerced, forced, or misled into joining the academy or submitting any personal information.
3. Understanding of Football as a Contact Sport
We acknowledge that football is a physical and contact sport. While KPFA shall provide basic medical care, first aid facilities, and appropriate protective training methods to minimize risks, we understand that injuries or health complications may still occur.
4. Limitation of Liability
KPFA, its staff, coaches, and affiliates shall not be held liable or responsible for any injury, illness, or health complication that occurs during or outside training, matches, tournaments, or academy activities. KPFA agrees to take reasonable precautions to ensure player safety.
5. Medical Fitness
We confirm that the player is physically fit and medically cleared to participate in sports activities. Any known health issues have been disclosed and submitted with relevant documentation.
6. Exclusive Rights to Representation
KPFA holds the exclusive right to represent, negotiate, and facilitate all exit, transfer, or professional football contracts for all players registered in the academy. This will be done in accordance with FIFA regulations and in the best interest of the player.
7. Training Compensation
In the event that the player signs a professional contract with another club after leaving KPFA, the academy shall be entitled to 5% training compensation from the player’s first professional contract in line with FIFA training compensation and solidarity contribution policies.
8. Code of Conduct and Compliance
All players are expected to abide by the academy’s code of conduct, rules, and regulations. Failure to do so may result in warnings, suspension, or dismissal from the academy without refund of any paid fees.
9. Use of Images and Videos
By registering, you consent to KPFA using the player’s photographs, videos, and performance highlights for academy promotion, scouting purposes, and public communication including social media and partner outreach.
10. Fee Structure and Refund Policy
All registration and academy fees are to be paid in accordance with KPFA’s payment structure. Fees paid are non-refundable except in cases where KPFA is unable to deliver the promised services due to internal academy issues.
11. Data Privacy
All personal information collected will be used solely for academy records, communication, and football development purposes. KPFA will not share sensitive information with third parties without prior consent.
12. Governing Law
This agreement shall be governed by the laws of the Federal Republic of Nigeria, and disputes shall be settled in accordance with Nigerian laws and applicable football regulatory frameworks.
